Our specially trained care teams can be accessed through a member's smartphone via our application or with a CareBridge supplied tablet.
Updates from our clinicians are provided to the member's Primary Care Physician (PCP) and health plan to ensure the member’s care is coordinated.
CareBridge’s clinicians perform comprehensive annual assessments and recurring virtual visits, close quality gaps, and generate care plans specific to each member’s needs.
